How do we collect information about you?
We obtain information about you in the following ways:
When you visit my website
When you enquire about our services via email and we reply to you via email, we cannot guarantee that your email, or my reply is 100% secure. It is important that you understand that no data transmission over the Internet can be guaranteed to be 100% secure. If you wish to send us any documents via email and have any concerns about confidentiality and the data contained within your documents, we are happy for you to password protect your documents before sending them to Hypnotherapy Plus. You can either provide us with your password in a separate email, or phone us and provide us with your password over the phone.
Over the phone
If you choose to make contact asking for a return phone call, Lincoln Counselling may collect information from you as a prerequisite for inviting you in for an assessment.
Face to face
When you attend a therapy sessions, we collect and record data from you in order to get to know you, understand you, and help you overcome your difficulties.
Once we agree on a day/time for you to attend a consultation session, we will ask you to complete a Client Details form-this form will ask you to provide me with personal information, including your name, date of birth, address and GP details
We may receive information about you from third parties we work closely with, including other health professionals and your health insurance company. Third parties including analytics providers provide me with information that helps me ensure my website is user-friendly and provides my website visitors with the information they seek. The only third parties who we will have reason to obtain information from in relation to your treatment are your insurance company and/or referring GP or psychiatrist. Your insurance company may refer you directly to me, and if they do, they will often provide Lincoln Counselling with your personal information. If your GP or consultant psychiatrist refers you to us for CBT, they may also write a referral letter which may contain both personal and sensitive information. If you have any concerns about whether the above third parties are GDPR compliant, please contact them directly. Lincoln Counselling will never knowingly obtain data about you from any third party without your knowledge or consent.

Lincoln Counselling is serious about data security and confidentiality and this document will explain what data we hold, why we hold it and how it is kept safe. Lincoln Counselling holds only information that has been supplied to us by our member. We will also explain your rights and how to opt in for marketing purposes.
What data is held?
We only hold data that has been formally submitted to us. This includes; Names, addresses, contact details (phone, email), specialism, signed documents (contracts) and professional photos, and assignments including case studies, presentations and personal journals
Where is my data stored?
Any data that has been submitted by a client to Lincoln Counselling will be stored in their respective folders which is found on local servers. This ranges from basic information, signed documents i.e. name, address and your personal needs should you submit them etc.
Retention of Information
The basis on which we hold or share information about you is outlined below and also the limits around confidentiality.
You have the right of access to information we hold about you and to amend or ask for deletion of information that may not be accurate or correct. Please contact us if you wish to view the data we hold about you. Lincoln Counselling will not hold your data for longer than is deemed necessary for the purposes of therapy or any other service we provide and in accordance with and following best practice and legal guidance. Is my data secure?
All data that we hold is located on local servers that can only be accessed by Lincoln Counselling staff who have been given permission. This means that only Lincoln Counselling staff can access this data. Furthermore, this information is safeguarded using appropriate anti-virus and firewall software that is regularly monitored and updated. We may have physical copies which are stored in a locked cupboard in our office.
What is my data used for?
Contact details are used so that we can contact individual members about updates. Signed documents such as contracts are saved so that we have proof that you as a client, comply with our rules and code of ethics.
Right of erasure
In accordance with applicable European legislation you can delete any information that we hold about you. You can request this by emailing: email@example.com
You have the right to exercise Your Data protection rights at any time and to request information as to whether and which Personal Data relating to you has been processed by us.
